Lyle House trial innovative new telehealth care scheme
At Lyle House Care Home we have been working closely with Wandsworth CCG and have been selected to pilot a new telehealth care scheme for care homes.
Telehealth care uses a range of devices to take readings such as blood pressure, weight, blood glucose and oxygen levels from residents. The results of the telehealth system monitoring are made available remotely to a nurse or doctor giving them instant access to a resident’s results.
The aim of the scheme is to reduce hospital admissions by identifying and acting on the early warning signs that someone’s condition is deteriorating.
The system will be ideal for monitoring people with long term or multiple conditions and bring a more coordinated approach to care between the care homes and NHS.
Staff are currently undergoing training and we hope that we’ll see the positive impact of the new scheme very soon.
